Why businesses record activity but lose the consequence

The gap between a decision and its outcome is where margin quietly disappears.

Most operational systems are good at recording activity: the quote was sent, the crew was dispatched, the membership was renewed. Almost none of them keep the consequence: what actually happened later because of that specific decision in that specific context.

The outcome, when it is recorded at all, lives in a different system from the reasoning that produced it. The estimate is in one place, the change order in another, the final margin in a third. Within weeks the connective tissue is gone, and the business is left with a warehouse of activity and no memory of what worked.

This is why expensive mistakes repeat, and why automation plateaus: a system that never sees what happened next cannot improve what it does next. Closing that gap means keeping the decision, the context, and the observed consequence connected. That connection is the missing infrastructure.
